Patient Portal

A member of our staff will activate you in the Patient Portal. Once you are activated you will receive an email with your user name and instructions to set a password

  1. LOG IN:
  2. Enter the User Name that was created for you by the practice
  3. Follow the Patient Portal prompts

The Patient Portal allows you to:

  • Directly access information contained in your electric health record
  • Update information regarding your current medical history
  • Receive normal lab results electronically
  • Ask questions about your care
  • Request prescription refills

Additionally, the Patient Portal can be used as a communication tool to update your Health Information including:

  • Current List of Medications
  • Allergies
  • Past Medical History
  • A Change in the name of the pharmacy where they want their prescriptions filled
